Your instructor has requested a course feedback survey from the Teaching Effectiveness Program. Instructors who choose this service want to do all they can to ensure course quality.
Please help by responding to this email to answer the following two questions. Your honest and constructive responses are greatly appreciated.

Survey responses will be submitted anonymously to your instructor and a random list of those who responded will also be submitted for credit (if you are receiving credit for this) in completing the survey. You will have until Thursday at midnight to complete the survey.
